8. A pre-operative history, physical examination and laboratory tests may be required prior to surgery. Your surgeon’s office will make these arrangements for you.

9. Please report to the Rye ASC one-hour prior to your appointment so that admission procedures and evaluation by the anesthesia staff may be accomplished.

10. Please bring all your insurance cards, proper ID and completed forms to the Rye ASC. The Rye ASC will expect you to be responsible for the facility and anesthesia charge if it is not covered by your insurance plan. This will include such costs as the charges for use of an operating room, equipment and personnel. Filling out the insurance and billing form in this packet will help us to complete and file the necessary claims with your insurance plan for this facility charge.

If you have any inquires regarding the Rye ASC fees, please feel free to contact our Billing Department at (914-681-3110) at least 3 days prior to the date of surgery.

Your surgeon will bill you separately for his/her professional fee. There may also be an additional bill for the services of an anesthesiologist if you require one.
